Thank you for your interest in Release and Renew Mental Wellness, LLC. Our practice provides comprehensive psychiatric evaluations, medication management, psychotherapy, and integrative mental health services for children, adolescents, and adults.

To begin the process of becoming a client/patient, please complete the pre-intake form below. This information helps our team determine the most appropriate services and provider for your needs.

What Happens Next?

Once your request is submitted:

  • You will be directed to our New Client Scheduling Portal.

  • You may review available providers and select the practitioner who best meets your needs.

  • You will be able to schedule your New Client Consultation Appointment.

  • All consultation appointments are conducted virtually via secure telehealth.

Important Information

To confirm your consultation appointment:

  • The New Client Consultation Form must be completed and submitted.

  • Any required intake forms and documentation must be completed prior to your appointment.

  • Appointments are not considered confirmed until all required forms have been received and reviewed.

After Your Consultation

Following review of your consultation information:

  • Our team will determine the most appropriate level of care and provider assignment.

  • Additional intake paperwork may be sent through the Client Portal.

  • If clinically appropriate for services, you will be scheduled for your comprehensive evaluation and treatment planning appointment.
